Abstract
We had previously analyzed about our ICU patients between March 1988 and February 1992, and now we have analyzed 2859 ICU patients admitted from March 1992 to February 1994.
Like previous study, we analyzed about the distribution of patients according to year, sex, age, admission route, duration of admission, and department-in-charge and he incidence of infection, operaion, ventilatory support, cardiopulmonary
resuscitation(CPR) and mortality based on the patient's admission records.
@ES The results of analysis are as follows;
@EN 1) The number of patients in 1992 was 1487, in 1993 was 1372. The number of male patients was 1727 and female patients was 1132.
2) The average duration of admission was 5.10 days in 1992, 5.26 days in 1993, and the overall average duration of admission was 5.17 days. The most common duration of admission was 3~5 days(37.8%).
3) The most common age group was 50~59 years923.8%). Most of the patients belonged to Internal medicine which was 43.0% of the total patients.
4) The number of patients, infection rate, CPR rate and mortality rate in medical ICU (MICU) were higher than in surgical ICU(SICU) which were 561%, 15.8%, 14.8% and 17.3%, respectively. But the ventilatory support rate and the operation rate
were
higher in SICU, which were 23.2% and 67.1%, respectively.
5) The rate of acquired infection during stay in ICU was 12.3% in 1992, 13.6% in 1993 and the overall infection rate was 12.9%. Total infection rate was 12.9%. Total infection rate was highest in the group of over 9 days medicine(54.6%). The
infection
rate was higher in non-operative group which was 16.2%, and most common infection site was pulmonary system962.1%)
6) The percentage of patients who received ventilatory support was 21.2% in 1992,20.6% in 1993. The highest rate of ventilatory support was in 3~5days of admission group(40.3%),in age group of 60~69 years(18.7%), and in the department of Internal
Medicine(38.3%). The ventilatory support rate was higher in non-operative group which was 21.0% and the highest ventilatory support rate in operative groups was cardiac and mediastinal groups was cardiac and mediastinal group which was 62.2%.The
most
common cause of ventilatory support was cardiovascular disease which was 30.4% of the total cases. The infection, operation, CPR and moartality rate in ventilatory support patients were 25.6%, 32.2%, 33.6% and 40.0% respectively.
7) The incidence of operation-related patients was 33.2% in 1992, 31.9% in 1993 and the overall incidence was 32.6%. The highest rate was in 3~5 days of admission group(43.6%), in age group of 50~59 years(24.3%), and in the department of Chest
Surgery(37.6%). The most common operation site was head and neck(41.0%). The infection rate in operative patients was 6.1%, ventilatory support rate was 20.7%, CPR rate was 3.3% and mortality rate was 4.2%.
8) The CPR rate was 12.8% in 1992, 9.7% in 1993 and the overall CPR rate Was 11.3%. The highest rate was in 1~2 days of admission group(50.3%), in age group of 60~90 years(21.3%), and in he department of Internal Medicine(58.0%). The success rate
of CPR
was 0.9%.
9) The mortality was 14.6% in 1992, 13.8% in 1993 and the overall mortality was 14.2%. The highest mortality was in 1~2 days of admission group(45.9%), in age group of 60~90 years(20.6%), and in the department of Internal Medicine(52.1%). The
most
common cause of death was CNS dysfunction(27.0%).
10) As conclusions, our hospital ICU showed the following characteristics in the last two years-higher frequency in male patients, in 3~5 days of the duration of admission, in over-50 years of age group and in the department of Internal Medicine.
The
incidence of the operation. ventilatory support, CPR and mortality showed decreasing tendency, while the rate of infection was increased.
